Hi Dries,
during my PhD I had good experience with the 24-well plates with glass bottom 
from http://www.glass-bottom-dishes.com/ (no comercial interest).
If you want to have individual coverslips try http://www.knittel-glaeser.de/ 
(again no com. interest).
They will produce custom diameters if you want.
Regarding the diameter I found it better to use 12mm, giving you a bit of 
space to lift the coverslip out of the well.
