Currently there is no "official" template for Google forms, but it's something we can add to the list.
Andria is right, it's fairly easy to take the form code and bring it into your own site, thereby removing the need to brand the form itself.
Andria, do you have any examples that you could show us?

If there is one I'd be giddy, but I don't think there is a umn template available. However, you can grab the source from a standard Google form and embed it on any page pretty easily... so long as creating additional pages on your site isn't too big of a deal.
There is a lot of extra code for required fields that can be stripped out. Just focus on the form and input tags. From there, you can set up a css to make it pretty and apply to future forms, too. Make sure to do the same for the Submit page or clicking the button will launch the default page from Google forms.

At today's Web Standards presentation, there were some questions related to a header and footer template in Google sites.
The best version to use is already available on Google sites. When you go in to create a new site, you're prompted to select a template to use. On that screen, click "Browse the gallery for more." This will give you a thumbnail list of all the templates that have been created for Google sites at the U.
The one that uses the approved web header and footer is the OIT Collaboration Site Template.
