OIT did a poll on the Research Series site at 
http://www.oit.umn.edu/research-series/ using Google Spreadsheets 
(audience is general public).  They also use a Google Spreadsheet Form 
for event registration (http://www.oit.umn.edu/news-events/register/) 
for events that OIT organizes, mainly for internal U audiences like 
faculty and staff.

In UR we did a sign-up form that saves the information back to a Google 
Spreadsheet.  You can view the sign-up form at 
http://community.umn.edu/housing-followup/.  Audience is the general public.
