Hi John,

We use Cobolt (http://www.cobolt.se/products.php#cw_visible), Coherent 
(http://www.coherent.com/Lasers/index.cfm?fuseaction=show.page&id=1007&loc=834), 
and Blue Sky Research (http://www.blueskyresearch.com/index.htm) diode 
lasers for our confocals. All are reliable and easy to use and they'll 
cover your 405 and 488 needs. The 488/491nm may also cover your 473nm 
requirement and the 405nm does excite CFP so may cover your 445nm 
requirement.
