Dear Martin,

Excellent points. We wanted to provide the survey to be anonymous (as
possible considering it is through the internet) to get as many facilities
as possible to participate. Anyone completing the document can voluntarily
provide personal information only if they want to receive an email when
results are ready or to be considered for the iPAD3 from the group of
respondents (see survey link: http://www.ideaelan.com/public/index.aspx). I
will send an email to the listserv when they are ready, anyway. Any
personal information or specific institutions partipating will NOT be
included in any reports. I hope this clarifies and my apologies for the
confusion.

I think a future version will include country of origin, directly. I will
check to see if the survey itself can determine the country of origin and
if possible, will break this down in final analysis. I recognize the
business models can vary differently internationally and your point is well
taken. We will brainstorm on how to sort out variation in costs of
service/instrument by country for a future version. Again, thanks for the
important feedback.
